Cash Pay Pricing
- New Patient Provider Visits: $150
- Follow-Up Provider Visits: $120
- Injection-Only Visits: $120 Testosterone Replacement Therapy++ (TRT) $60
- This is an Intramuscular Injection-TRT Regimen of:
- Take-Home/To-Go Pre-filled syringes are available for self-administration starting at $40 per week and include: Testosterone, Vitamin D3 and Vitamin B12. (CoQ-10 is not included for self-administration).
- Testosterone, Vitamin D3, Vitamin B12 and CoQ-10 (for in-clinic administration)
Patients are required to be evaluated in-clinic every 3-4 weeks. Above pricing will apply.
Insurance Patients (Injection-Only Visits):
- $35-$60 Pending Coverage (Some insurance plans may not cover Injection-Only Visits. A $35 payment is due at time of service with the total out-of-pocket cost up to $60 per visit. Patients will be informed of payment adjustments based on coverage. Please be advised that insurance claims can take several months to process. Patients are responsible for all treatments provided. If coverage is denied by insurance for any visit, patients are subject to cash-pay pricing going forward.)
Payment Requirements:
- All Provider Visits will require payment in full at the time of scheduling.
- Services such as cash-pay labs and prescription medication orders, will require payment in advance or at the time of service.
- As extensive time is set aside per visit, we will require 48 hours cancellation notice to receive a full refund.
